Before You Arrive
Thank you for choosing Dr.Yuri Lewicky and Northern Arizona Orthopaedics and the Summit Center for your Orthopaedic needs. Please arrive 20 minutes early in order to fill out any additional paper work. You may call (928) 226-2900 prior to your arrival to see if the Doctor is on schedule and adjust your timeline appropriately. If you must cancel your appointment please give us 24 hour notice beforehand. You will need to bring with you proper attire for the body part being examined. For example, a pair of shorts if you are being seen for a hip or knee problem and a tank top for women with shoulder problems. If you have seen any other Orthopaedic Surgeons for the problem that we are seeing you for, we will need copies of their clinic notes, operative notes and radiographic studies, (XR, MRI, CT, etc..). We will also need to copy your Insurance Card at your first visit. Additionally please bring with you the Co-pay/Payment (if required) and the Referral (if required by your insurance company). Our goal is to make your office visit a pleasant experience that is less time consuming and more informative.
What to Expect
When you arrive, please understand that you will be asked many questions by many people, some of the questions will be ones that you have already answered in person or on paper. Please be patient and understand that this repetition is necessary given the current medical environment. If you are frustrated understand that often times we are too. Though you may have given a complete written answer, hearing it directly from you, the patient, cannot be substituted for. At your visit you will be asked to focus on one particular problem, this facilitates your visit and allows for less confusion when your treatment is prescribed. If you should have another Orthopaedic issue that needs to be addressed then this should be scheduled for another visit. The doctor may request that some additional Common Orthopaedic Studies be performed; some may be available at the Summit Center while others may require that you go to an outer lying facility and return for another appointment. Please understand that unforeseeable events do occur and staying on schedule may not always be possible. Rest assured that the Doctor will spend the time necessary to adequately diagnose and treat your problem.
